Virtual Networking Events
Connection is key!
In lieu of exhibitor tables and networking lunches, we are offering virtual networking events at HLCON for the first time ever.
​
Mingle with colleagues, peers and new friends in these virtual "hallways" that provide a social gathering spot in between learning sessions. Hallways are hosted by guest community leaders. Take time to meet and greet and learn about new resources and programming from local organizations.

A safe space for Community Health Workers, Navigators, Promotores, & Promotoras to openly discuss common stressors in their work. It is a welcoming space to seek emotional support, vent, compare notes, share resources, and just make a human connection with others in the CHW field. If you are a CHW or thinking about becoming one, please join us! There is strength in our numbers and love in our hearts. You are not alone!

Grow Healthy Together Pathways HUB
Host: Sophia Velasquez, LPC, HUB Manager
The Grow Healthy Together Pathways Community HUB is a data driven communitywide system that connects low-income residents to needed medical care and social services to improve health outcomes. Key to the program are Community Health Workers (CHWs), who provide care coordination services and are employed by numerous clinical, social service agencies and other organizations throughout the community.
